A Patient Centered Medical Home is not a home or building. It is a team approach to providing comprehensive health care in a high quality and cost effective manner. As a patient you are the most important person in the health care system. Being part of a medical home provides total health care with a team that includes health care professionals, family members or friends (if you wish) and you. Medical assistants, social workers, dietitians and nurse practitioners will be a part of this health care team. They will be there to answer your calls and questions, reminders about checkups and tests, and coordinate your care with specialists.
Learn about caring for yourself understand your condition and what you can do to stay as healthy as possible. Set obtainable goals and work hard to reach them. Talk openly with your team, if there are problems discuss them with your team to find a solution. If the plans or goals are not working, speak up. Feel free to share your good and bad past experiences.
Our patient centered medical home allows you to call our office first. In our office, we strive for personalized care that is safe and scientifically valid, with a focus on prevention to keep you healthy.We are old style family doctors, backed by teams, a network of specialists, and modern technology. Our focus is to care for you as an individual in the best way possible.
Your physician and your team know you and you know us. We will address your needs in our office or connected with other services outside our office. We coordinate your health care with trusted specialists and other health care providers. We will help you get appointments and will ensure that your other providers have all the information they need. We use a safe and secure electronic medical record (EMR) to maintain your information and to communicate with in network hospitals on lab tests, screenings and other important information. If you go to an out of network hospital, our office makes all attempts to obtain your information. The new web-based patient portal will allow us to view your health information and communicate with your team through email.

After Hours Care
For urgent questions after office hours, your call will be received by our answering service and a licensed Registered Nurse/Nurse Practitioner from the night nurse triage will return your call within 1 0 to 30 minutes depending on the urgency of your situation. The Nurse/Nurse Practitioner will advise you and if necessary, direct you to the nearest health care facility for further care.
